1. What is your date of birth (dd/mm/yyyy)? Please note that you must be over 18 years of age to use my coaching services.

4. How much would you like to weigh?

5. Why do you want to lose weight now?

6. What is the highest (non-pregnant) weight you have ever been?

7. When were you at your highest weight ever?

8. Have you ever weighed less than your current weight? If so, what is the lightest you have ever weighed? When and under what circumstances did you weigh that much?

9. Have you tried to lose weight in the past? If so, which strategies were the most effective for you? Which strategies were the least effective for you?

My telephone coaching hours are Monday to Friday 9.30 am to 5.00 pm and 9.00 pm to 10.00 pm as well as Saturdays 1.30 pm to 5.00 pm and 9.00 pm to 10.00 pm Australian Eastern Standard Time.
